To enter the Primordial Vault where the quest is accepted, you must first find the 3 Vault Fragment collectibles in this region.
On the rooftop of a gas station on a dining table. Pull yourself up the grappling point at the front of the roof near the street.
Look for the distinctive yellow grapple points around the gas station structure. The fragment is well hidden among some debris on the dining table.
Found inside a tunnel. Drive halfway through the tunnel, there you find a grate on the floor that you can pull out. When you use ECHO-4's scan functionality it will be marked in green color to see it more easily. After pulling out the grate, drop down to reach the vault fragment.
Use your vehicle's headlights to better see the grate in the dark tunnel. The scan function will make it glow bright green for easy identification.
Found on the side of some rocks, out in the open, easy to reach. This is the most straightforward fragment to collect.

When you approach, a stone pillar will come out of the ground where you can insert the Carcadia Burn Vault Key. Then a purple portal appears, step through it to enter the Primordial Vault.
On the inside you can accept a side quest, then fight through 3 arenas and beat the Vault Guardian boss at the end.
Defeat all enemies in the Primordial Vault. There are 3 arenas with waves of enemies. After each arena you must step on a yellow ramp to be catapulted to the next arena.
Use cover effectively in each arena. The enemies come in waves, so manage your ammo and abilities carefully. Each arena has different enemy types, so adapt your strategy accordingly.
In the fourth arena you will encounter the Vault Guardian. Defeat him. Afterward go through the purple rift and collect your reward. Then exit through the purple rift orb to complete the quest.
Radix has several attack patterns. Watch for his tell signs before big attacks. Aim for critical spots on his body. Use corrosive or shock weapons for maximum effectiveness. Keep moving to avoid his area-of-effect attacks.
